Personalized Support for Your Pregnancy Journey
Connect with a Perinatal Community Health Worker and reclaim your care with confidence.
Whether you’re pregnant, postpartum, or somewhere in between, your experience matters, and so do your choices. At ADRC’s Rooted Health Front, we offer 1-on-1 support with trained Perinatal Community Health Workers (PCHWs) who walk alongside you, not ahead of you.
This isn’t one-size-fits-all care. It’s your plan, your pace, your power.

What Is a Perinatal Community Health Worker?
Our PCHWs are community-based advocates trained in reproductive health, systems navigation, and trauma-informed care. Many of them are parents, doulas, or birth workers themselves; people who understand both the challenges and the beauty of the journey you’re on.
We’re here to listen, offer tools, and make sure your voice stays at the center of every decision.
What You Can Expect
Our PCHWs are here to help you:
Build a Perinatal Plan that reflects your values, traditions, and needs
Navigate pregnancy options with clarity and support — no pressure, no judgment

Create a care strategy rooted in informed consent and shared decision-making
Explore cultural birthing traditions and rites of passage that resonate with you
You don’t have to figure this out alone. We’re here to help you feel grounded, equipped, and supported every step of the way.
